Legal Literacy Support for CHC – Islington– Up to £425PD (Outside IR35)

Role Details:

The project is being set up to ensure that we can deliver  proportionate reviews; assessments and follow up actions in relation to the statutory functions part of Care Act as well as to ensure that CHC section 117 and people moving through transition can all receive the correct support and negotiations to maximise opportunities around other funding and shared accountability where a person is subject to sect 117 aftercare and CHC considerations I t also assists the assessors to proceed where they are able to for appeal or negotiating  care package costs where CHC/117 isn’t the appropriate framework to follow

Requirements

Procedures for delivering Targeted reviews

Process and summary of workflows when delivering timely and targeted reviews for people whose needs could include:

•            Continuing Health Care.

•            Moving through Transition &

•            Section 117 aftercare.

•            High-Cost packages.

Legal literacy

All work will be supported throughout via legal literacy and the offer of  raised awareness and support in practice via additional support coming into the Project for this purpose . 

Administration

Business support functions will assist in retaining records for purpose of regular evaluation of progress 

Legal

Is in place to support any queries or advice needed in relation to progressing appeals or any specific individual cases of concern

Finance

Will support the projects and develop a regular check in of progress and reporting in terms of savings and timelines for allocated reviews and CHC work

Where CHC is not successfully achieved, but packages of care remain high, the process will be inclusive of negotiating jointly funded packages of care with the relevant ICB(CCG) as part of the CHC/review processes undertaken throughout the project. All of this will be reported regularly via the finance reporting process and the contributions delivered via the project. The live tracker will be updated as things change and a weekly update meeting will take place to keep the project and workstreams on track. 

Commissioner

Will support the team in terms of negotiations around Care Cube and Provider conversations to maximises the opportunities to realise benefits

Brokerage

Will support to place all agreed package amendments into CONTROC

Performance

Team will continue to track performance as part of this project, also ensuring that the improvement around reviews is reported weekly.

CHC -this is being progressed as recognition that several people within ASC are potentially eligible for CHC status and are not currently in receipt of free services and support.

ILDP Staff .  

We have appointed 2 additional staff to undertake the  CHC process and are asking for an additional 2-3 business case to follow.  We have also appoint Ralliat as  a Senior Practitioner  to lead on CHC and have secured the services of an external expert 2 days  a week for 6 months to support the  process in LD as a de facto team manager working remotely to progress chase and QA .
